Package and Prepaid Treatment Terms:

• Valid for one person only and can’t be shared with other individuals unless gifted in entirety. 
• Valid for one year from purchase date.
• May require an establishing care appointment or brief consult prior to first treatment to make sure treatment is safe and effective for the patient.
• If the patient chooses to discontinue treatment, the remaining balance will be issued as clinic credit for other services (supplements/meds not included) and must be used within one year of original purchase date. 
• If our clinic discontinues services or you are advised against further treatment, the patient will receive a refund for any unused treatments.
• In general only successfully administered treatments count toward the total purchased. If a treatment cannot be completed—for example, due to difficulty placing an IV line or an early stoppage—it may not be counted. EBOO that has exceeded 30 minutes or more of treatment is considered a successful treatment.
• In cases of certain patient emergencies—including but not limited to relocation, a death in the family, a natural disaster (such as wildfire, flood, or other catastrophic events) that forces the patient to move, or a significant change in health status (such as the development of a new medical condition or a change in treatment plan recommended by another healthcare provider)—or any other unforeseen and significant hardship that prevents the patient from completing treatment, a refund for unused treatments will be provided.
• Prices subject to change and packages cannot be combined with other discounts.
